Small Business Service Bureau, Inc. (SBSB) is an affiliate of Carroll Enterprises,
Inc., a privately held corporation which provides products and services through
its wholly-owned subsidiaries: the Small Business Insurance Agency, Inc. (SBIA),
Pivot.com, Sterlingbridge, Group Health & Benefit Administrators, Inc. (GHBA), and
Direct Response Marketing, Inc. (DRM). This disclosure statement is intended for
all users of SBSB.
SBSB insurance program availability varies by state. Discounts are not available
for insurance products. Insurance is regulated by each state. Not all products are
offered in all areas. Membership in SBSB is not required to purchase insurance,
nor does it mean automatic approval for insurance or services. SBSB insurance products
and programs are offered through its affiliate, Small Business Insurance Agency,
Inc. (SBIA), a national general insurance agency licensed in all 50 states.

of 2006.
